GHCA Support Coordination Services
Coordination of Supports (L1)
This level involves basic coordination to help participants understand and use their NDIS plan effectively. It includes connecting with service providers, arranging supports, and ensuring services are delivered as planned. Additionally, it focuses on providing guidance to ensure participants access and manage their supports efficiently while addressing any immediate needs or challenges.
Coordination of Supports (L2)
At this level, support coordination includes everything in Level 1, plus additional capacity-building activities. This means helping participants develop the skills to manage their own supports more independently in the future. It involves more complex support planning, including connecting participants to community and mainstream services and resolving more intricate issues.
Specialist Support Coordination (L3)
This level offers intensive support for participants with complex needs. It includes all aspects of Level 2 support coordination, with an added focus on managing high-level risks and navigating complex situations. Specialist Support Coordinators work with participants to address significant barriers, ensure safety, and resolve critical issues impacting their support and well-being.
